What Services are Available?
- Purchase, repair, adjustment and replacement of device, component or supplement (with specified conditions)
- If the device is lost, stolen or damaged through negligence, its cost is not covered
Procedure for Getting Access
- Provision of a written medical prescription by an authorized physician, which must include a statement of the physical impairment and resulting incapacity, necessity of obtaining a device, and recommendations for device use
