Eligibility
- Programs where all ages are eligible
- Cerebral Palsy
- Must be a person with cerebral palsy in Ontario
- Valid Ontario health card
- Individual Member of the Ontario Federation of Cerebral Palsy
What Devices are Covered?
- Communication and writing/reading aids
- Orthotic devices
- Wheelchairs, positioning and ambulation aids
- Home accessibility aids
- Bathing and washroom aids
- Modifications to electronic aids for daily living
- Some may be ADP or non-ADP approved devices
What Services are Available?
- Funding assistance of equipment or material being ordered or purchased
- Labour/installation, delivery, therapy or assessment costs are not funded
- Funding for one piece of equipment every two years up to $2000 per item per application
Procedure for Getting Access
- Completion of the Assistive Devices Funding Program Application Form
- ADP approval documentation (for ADP approved items)
- Health professionals (i.e. occupational therapist, physiotherapist, medical practitioner) authorization for requested items
- Two quotes from Ontario vendors where applicable
- Letters from other sources of funding that have been accessed or denied
