Eligibility
- Programs where all ages are eligible
- Ontario resident
- Valid Ontario health card
- Have a physical disability of six months or longer (income is not considered)
- There are specific eligibility criteria that apply to each device category
- Not eligible for ADP if the applicant qualifies for or are receiving financial support for the same equipment or supplies from the Workplace Safety and Insurance Board, OR is a Group "A" veteran and already qualify for or are receiving financial support from Veterans Affairs Canada for the same equipment or supplies
What Devices are Covered?
- Mobility aids
- Hearing aids and other devices
- Communication aids
- Visual aids
- Artificial eyes and facial prosthetics
- Custom orthotic braces, compression garments and lymphedema pumps
- Prosthetic breasts or limbs
What Services are Available?
- Cover 75% of the cost for equipment and supplies
- Does not cover repair costs
- Care and maintenance is client responsibility
- May contribute to cost of equipment replacement if the applicant's condition changes or the equipment is no longer under warranty and cannot be repaired at a reasonable cost
Procedure for Getting Access
- Medical specialist or general practitioner provides a diagnosis
- An authorizer assesses the specific needs of person and prescribes the appropriate equipment or supply
- Vendor sells equipment or supplies to client
