- Be a resident of Saskatchewan
- Possess a valid Saskatchewan Health Services Number
- Must have equipment requisitioned by a medical professional with appropriate authority
- Program benefits are provided to people residing in the community

What Services are Available?
- Loan of equipment (depending on individual health coverage)
- Maintenance and repair for program-owned equipment are covered
- Installation or funding of installation of equipment in the applicant's residence is not covered
- Personal Care Home and Special Care Home residents are eligible only for walkers, wheelchairs and cushions
- Patients in an acute care facility are not eligible for Special Needs Equipment benefits except as part of a definitive discharge plan
Procedure for Getting Access
- A health professional with authority to requisition a specific type of equipment (refer to the website guide for the individual with requisitioning authority) must complete a Special Needs Equipment Requisition Form on behalf of the client and send it to one of the five Saskatchewan Abilities Council Special Needs Equipment depots
- Clients must be referred for the service by an authorized health care professional, and the service must be obtained in Saskatchewan
