Eligibility
- Programs where all ages are eligible
- Victim must have sustained property damage, or been injured or died as a result of a crime committed in Quebec
- Victims of crimes that took place in Quebec, but who do not live in the province may also submit an application
- Victims must: provide documentation of the crime, demonstrate that injury or death resulted directly from the crime, supply a document detailing the nature of the injuries caused by the crime, and may have sustained either a physical or psychological injury
What Devices are Covered?
- Assistive device must be related to the after-effects of the criminal act
What Services are Available?
- Assessment, maintenance, repair, and replacement of equipment
- All care, treatment or equipment needed are provided so long as there is proof that it is related to the criminal act
Procedure for Getting Access
- A physician or recognized health care provider must fill out the appropriate Rapport Medical or Attestation Medical Form, which must include proof of injury, diagnosis, and prescription information
- As long as it can be shown that the equipment is required for the compensation of deficits related to the injury or disease, it will be covered (exception of wheelchairs, which are covered under the Régie de l'assurance maladie du Québec)
