Eligibility
- Programs where all ages are eligible
- Clients of the Social Development Department and their dependents
- Individuals who have special health needs and who qualify for assisted health care under Section 4.4 of the Family Income Security Act and Regulations
- Must have one of the following: valid white Health Services Card showing “SUPPLEMENTARY” in the BASIC HEALTH ELIGIBILITY section, or “CEQUIP” in the ADDITIONAL HEALTH ELIGIBILITY section; or a valid yellow Health Services card with a “Y” under the OTH in the VALID ONLY FOR box; or an “X” under SUPP in the VALID ONLY FOR box

- Manual wheelchairs
- Power wheelchairs
- Four-wheeled scooters
- Rehabilitation strollers
- Seating and positioning aids components
- Equipment may be provided new or recycled
What Services are Available?
- Modifications and repairs
- Equipment rentals
Procedure for Getting Access
- A prescription form is required from an occupational therapist or physical therapist
