- Adults (18 to 64)*
- Resident of New Brunswick
- 19 to 64 years of age
- Have a long-term disability (does not include medical condition that does not result in long term disability, or services required to address drug, alcohol, nicotine or gambling addictions)
- Require disability related supports to address unmet needs and to establish or maintain living arrangement in community, to assist or enhance capacity of natural support networks to provide support in community, or to help participate in community
- Require disability related supports to address unmet needs and to assist or enhance the capacity of the applicant's support networks to help the applicant to participate in community
What Devices are Covered?
- Technical supports and assistive devices not covered under other programs
What Services are Available?
- No cost for general facilitation services
- Service delivery model allows self-management of plan or request support of a social worker
- Clients requesting ongoing support of a social worker will meet with the social worker todevelop a case plan and receive regular contact, advocacy and support
Procedure for Getting Access
- Application may be completed by the applicant, help from an independent facilitator, help from family or friends, or help from staff of the Social Development Department
- Financial assessment for funding assistance
- Needs are based on a personal disability support plan through their independent facilitation service
