Eligibility

- Older Adults (65 and over)*
- Services offered to low-income individuals 65 years or over, and to Nova Scotians 19 years or older living with an acquired brain injury
- Resident of Nova Scotia with valid Health Card
- Live alone
- Annual net income of less than $22,125
- Have a history of recent falls
- Use a cane, wheelchair or walker
- Requirement for Home Care Services that will extend beyond 90 days
- Sign agreement with Department of Health and Wellness
- First Nation individuals on reserve are eligible for program if they are not receiving Income Assistance (Aboriginal Affairs Northern Development Canada)
What Devices are Covered?
- Personal Alert Device Personal alert emergency response system (i.e. LifeLine, Project Lifesaver)
What Services are Available?
- Assessment for eligibility
- 24 hour emergency support and alarm system
- Maximum financial assistance is $480 per year, which includes one-time set-up fees and recurring monthly charges
Procedure for Getting Access
- Apply by calling Continuing Care services
- Submission of invoices to Nova Scotia Health Authority
- Must be authorized by a care coordinator from the Department of Continuing Care
