Eligibility
- Programs where all ages are eligible
- Current regular bed resident of the Department of Health licensed or approved long term care
- Requires equipment on the program's Covered Equipment List
- Assessed by an Occupational Therapist or Physiotherapist as requiring the specified equipment
- Have the request reviewed and approved by Continuing Care
What Devices are Covered?
- Specialized mattresses and accessories
- Resident specific, specialized bariatric beds
- Power and manual wheelchairs with accessories
- Resident specific specialized transfer aids (i.e. sliding sheets, trapeze bars)
- Basic positioning chairs
- Customized 2 wheeled walker with glide tips/skis and rollator walkers, with accessories
- Resident specific slings
- Bariatric commodes
What Services are Available?
- Customized equipment to specifications
- Delivery, set up, maintenance, pick up, and recycling of equipment
- User may pay monthly rental fee
Procedure for Getting Access
- Professional assessment and submission of a request form by a physiotherapist or an occupational therapist
- Review and approval of requests by Continuing Care
