What Services are Available?
- Equipment loans are free of charge
- Loan out only one unit per client (i.e. will not be loaned more than one of the same piece of equipment)
- No costs associated with delivery and pickup of equipment, and service due to regular wear and tear
- Costs are associated with training, calibration, and disposable components to pieces of equipment
Procedure for Getting Access
- Completion and submission of the Client Enrollment Form
- Member of the client's healthcare team must submit a prescription to the ALS Society via the Prescription Form
