Eligibility
- Programs where all ages are eligible
- Amyotrophic Lateral Sclerosis
- Proof of diagnosis of amyotrophic lateral sclerosis (ALS), primary lateral sclerosis (PLS) and/or motor neuron diseases
- Registered with ALS Canada
- Accessed all other sources of financial assistance through private insurance
- Accessed Assistive Devices Program (ADP) and other government and non-government sources before applying
What Devices are Covered?
- New and recycled electric/power wheelchairs and electric/power tilt/recline wheelchairs
What Services are Available?
- ALS Canada will cover costs of most repairs and return of equipment
- Client pays installation costs associated with equipment and delivery
- Other funding limitations may also apply
Procedure for Getting Access
- Assessment by a registered healthcare professional (i.e. Occupational Therapist, Physiotherapist, or Registered Nurse) and submission of the Equipment Funding Assistance Request Form
- Quote from the Centralized Equipment Pool and client's notice of assessment must be submitted
- Centralized equipment pool operated by Motion Specialties and funded by Ontario's Ministry of Health and Long-Term Care
