Eligibility
- Programs where all ages are eligible
- Amyotrophic Lateral Sclerosis
- Proof of diagnosis of amyotrophic lateral sclerosis (ALS), primary lateral sclerosis (PLS) and/or motor neuron diseases
- Registered with ALS Canada
- Accessed all other sources of financial assistance through private insurance
- Accessed Assistive Devices Program (ADP) and other government and non-government sources before applying
What Devices are Covered?
- Bathroom aids: commodes, bath lifts, sliding tub transfer benches
- Mobility aids: wheelchairs (transport, manual, and power) scooters, walkers
- Lift and transfer devices: mechanical or power lift, ceiling track lifts, free standing lifts, ramps, lift chairs, superpoles
- Bedroom aids: hospital beds, hospital and pressure relief surfaces
- If an item from the list is not available and is considered basic and essential, ALS Canada may provide funding assistance for purchase, lease, or rental cost
What Services are Available?
- Basic and essential equipment on loan to clients for however long it is needed
- Provides one of each device per client
- ALS Canada will cover costs of most repairs and return of equipment
- Client pays installation costs associated with equipment and delivery
- Other funding limitations may also apply
Procedure for Getting Access
- Assessment performed by a registered healthcare professional (i.e. Occupational Therapist, Physiotherapist or Registered Nurse)
- Loan Equipment Request Form to be completed and submitted by the healthcare professional
